Description
- Coordinate support services such as transportation, housing assistance, or financial aid for patients and families navigating the healthcare system.
- Facilitate communication between healthcare providers, patients, and families to ensure understanding of care plans and available resources.
- Maintain documentation and case records to support continuity of care and compliance with institutional policies.
- Advocate for patient rights and access by working with insurance providers, legal aid, and community organizations.
- Develop and manage resource directories and referral networks to connect patients with external support programs and social services.

Additional Skills & Qualifications
• Requires a minimum of a Master’s degree in Social Work, Clinical Psychology, or Psychology. A concentration in Gerontology, Pediatric, Public Health, Substance Abuse, Mental Health or other related fields is preferred
• Requires a minimum of five years of clinical work experience in the above field
An active, unrestricted Licensed Clinical Social Worker license substitutes for two years of work experience
Job Type & Location
- This is a Contract to hire position based out of Santa Barbara, CA.
- Hybrid schedule
- In Santa Barbara office 1–2 times per month
- If outside Ventura/SB/SLO areas but still in CA: office visit 1x per quarter
