Job Description
As a Social Worker, you will coordinate support services such as transportation, housing assistance, and financial aid for patients and families navigating the healthcare system. You will facilitate communication between healthcare providers, patients, and families to ensure understanding of care plans and available resources. You are responsible for maintaining documentation and case records to support continuity of care and compliance with institutional policies. Your role also involves advocating for patient rights and access by collaborating with insurance providers, legal aid, and community organizations. You will develop and manage resource directories and referral networks to connect patients with external support programs and social services.

Job Type & Location
This is a Contract position based out of Santa Barbara, CA.
Pay and BenefitsThe pay range for this position is $39.50 - $39.50/hr.
