Job Description
Join a reputable organization specializing in long-term care, skilled nursing, acute care, and hospice services as an Intake Nurse. This office-based role is ideal for nurses looking to transition from hands-on patient care to a more administrative setting. You will play a crucial role in coordinating referrals, verifying and entering physicians' orders, and identifying the required level of care for patients.
Responsibilities
- Coordinate patient referrals and verify physicians' orders.
- Identify the appropriate level of care required by patients.
- Review documentation to determine facility placement.
- Coordinate orders with pharmacists.
- Collaborate closely with referring hospitals, physicians, skilled nursing facilities, patients, and families.
Essential Skills
- Licensed Practical Nurse (LPN) or Registered Nurse (RN) licensed in the State of Utah.
- At least 1 year of experience, preferably in a skilled nursing facility.
- BLS (Basic Life Support) certification.
- Experience in referral coordination, skilled nursing, and long-term care.
Additional Skills & Qualifications
- Experience with MatrixCare or Point of Care Electronic Medical Records (EMR).
Work Environment
Work in a professional office setting with a business casual dress code. The team consists of 32 office staff, with 9 members on the immediate team. The schedule is a rotating Monday-Friday shift between 8:00 AM and 7:00 PM, with a weekend shift and on-call duty every 6-8 weeks. The role is based in an office building, separate from the care facility itself.

Pay and BenefitsThe pay range for this position is $32.00 - $32.00/hr.
