Job Title: Intake Nurse
Work Environment
This position operates on a rotating Monday through Friday schedule between the hours of 8am and 7pm, with weekend work every 6-8 weeks and on-call duties. The dress code is business casual. You will work in an office setting with a team of 32, including 9 on your immediate team.
Job Description
We are seeking a dedicated Intake Nurse for a large long-term care, skilled nursing, acute care, and hospice organization. In this role, you will be responsible for coordinating referrals, verifying and entering physician's orders, identifying the required level of care for patients, reviewing documentation for facility placement, and coordinating orders with pharmacists. You will work closely with referring hospitals, physicians, skilled nursing facilities, patients, and families. This is an office position with no hands-on patient care.
Responsibilities
- Coordinate referrals and manage related documentation.
- Verify and enter physician's orders accurately.
- Identify the required level of care for each patient.
- Review and determine appropriate facility placement for patients.
- Coordinate medication orders with pharmacists.
- Collaborate with referring hospitals, physicians, skilled nursing facilities, patients, and families to ensure smooth transitions.
Essential Skills
- LPN or RN license in the State of Utah.
- At least 1 year of experience in a skilled nursing facility is preferred.
- BLS certification is required.
- Proficiency in referral coordination and skilled nursing care.
Additional Skills & Qualifications
- Experience with Matrix Care or Point of Care EMR systems.
- Familiarity with long-term care practices.

This is a Contract to Hire position based out of Woods Cross, Utah.
Pay and Benefits
The pay range for this position is $32.00 - $32.00/hr.
