Job Description
This office-based Intake Nurse role supports a large long-term care, skilled nursing, acute care, and hospice organization by coordinating referrals and ensuring accurate, timely intake of patients into appropriate levels of care. The position focuses on reviewing documentation, verifying and entering physician orders, and collaborating with hospitals, physicians, skilled nursing facilities, patients, families, and pharmacists to facilitate smooth transitions into care settings. This is a non-clinical, no hands-on patient care role, ideal for nurses who want to leverage their clinical experience in a structured office environment.
Responsibilities
This role is based in an office building in Woods Cross, Utah, separate from any care facility, and consultants will work only in the office environment. The setting is business casual, with a professional yet relaxed dress code. You will work alongside approximately 32 colleagues in the office, with about 9 people on the immediate intake team, fostering a collaborative and supportive atmosphere. The schedule follows a Monday–Friday rotating shift between 8:00 AM and 7:00 PM (5x8), with a requirement to work a weekend approximately every 6–8 weeks and to take on-call duty on a similar rotation, with days off adjusted accordingly. Work is performed at a computer using electronic medical record systems such as MatrixCare or Point of Care, focusing on documentation, coordination, and communication rather than direct patient care. The position is structured as a 13-week contract with a strong likelihood of extension based on business needs and performance, and there may be opportunities to move into a direct role over time. This environment is well-suited for nurses seeking a predictable office schedule, no hands-on patient care, and the opportunity to use their clinical knowledge in a non-clinical setting.

Responsibilities
- Coordinate incoming referrals from hospitals, physicians, skilled nursing facilities, patients, and families to ensure timely and accurate intake.
- Verify and enter physician orders into the electronic medical record system with a high level of accuracy and attention to detail.
- Review clinical documentation to determine the appropriate level of care required by each patient.
- Assess documentation to determine the most suitable facility placement for patients based on clinical needs and care criteria.
- Collaborate closely with referring hospitals, physicians, and skilled nursing facilities to clarify orders and resolve discrepancies.
- Coordinate medication orders and related information with the pharmacist to support safe and appropriate medication management.
- Communicate clearly and professionally with patients and families to explain placement decisions and next steps in the care process.
- Maintain thorough, accurate, and timely records in the EMR system, including MatrixCare or Point of Care, as applicable.
- Follow established intake protocols, policies, and regulatory guidelines to ensure compliance and high-quality service.
- Participate in a rotating weekday schedule and periodic weekend and on-call coverage as assigned.
- Collaborate with the immediate intake team and broader office staff to support a smooth, efficient intake workflow.
- Protect patient confidentiality and handle sensitive information in accordance with privacy and compliance requirements.
- Active LPN or RN license in the State of Utah.
- At least 1 year of experience, preferably in a skilled nursing facility or similar post-acute care setting.
- Basic Life Support (BLS) certification.
- Demonstrated experience in referral coordination within healthcare or a related environment.
- Knowledge of skilled nursing and long-term care workflows and documentation requirements.
- Ability to review clinical documentation and identify appropriate levels of care and facility placement.
- Strong data entry and documentation skills, with accuracy in verifying and entering physician orders.
- Effective verbal and written communication skills for working with hospitals, physicians, facilities, patients, and families.
- Ability to work a Monday–Friday rotating schedule between 8:00 AM and 7:00 PM, including periodic weekend and on-call coverage.
- Proficiency with electronic medical records (EMR) systems.
- Experience working in a skilled nursing facility is strongly preferred.
- Experience with MatrixCare or Point of Care EMR systems.
- Prior experience in long-term care, hospice, or post-acute care environments.
- Strong organizational and time management skills to handle multiple referrals and tasks simultaneously.
- Ability to work effectively in a collaborative office team of clinical and non-clinical staff.
- Comfort transitioning from hands-on clinical roles to a non-clinical, office-based nursing position.
- Reliability and consistent attendance to support continuity of intake operations.
