Incident Management Coordinator
Reports to: Incident Management Coordinator
Location: 1603 Court Street, Syracuse, NY 13208
Pay Rate: $24-$26 Hourly
Schedule: Monday Friday: 8:00 a.m. 4:00 p.m.

Our focus is supporting individuals by ways of residential housing, mental health and clinical services, and immersing our participants in community engagement. Working closely with the Office for People with Developmental Disabilities and the Office of Mental Health, we support 27 housing locations and 3 administrative buildings.
About the Role:
We are looking for a proactive employee to oversee and coordinate the reporting and management of incidents involving individuals with developmental disabilities or mental health diagnoses.
Responsibilities:
- Process, monitor, and review, all reportable and non-reportable incidents in conformance with OPWDD Part 624 of 14NYCRR and OMH/Mental Hygiene Law Section 29.29
- Review and verify incident documentation for completeness and accuracy
- Assign internal investigators to incident investigations transmitted by the NYS Justice Center
- Conduct investigations transmitted from the NYS Justice Center, as needed
- Responsible for the direct supervision of Quality Enhancement (QE) Specialists
- Review incident investigations completed by QE Specialists to ensure compliance and quality standards are met
- Monitor and track Agency OPWDD and OMH reportable incidents
- Provide administrative and management support and services relative to incident management to other non-for-profit organizations under contract with Access Management Services LLC
- Monitor the implementation of Special Review Committee recommendations; report any issues
- Ensure compliance with Care Coordination Organizations
- Coordinate with Quality Enhancement staff to ensure on-going compliance with federal and state incident management requirements
- Ensure compliance with federal, state, and local employment laws and regulations, as well as company policies
- Perform Other Duties as Assigned
Education/Experience:
Associate degree in Human Services or related field and at least 2 years working in an OPWDD setting, required. Minimum of 1 year of experience as an incident investigator, required.
Conditions of Employment:
Must have a valid NYS Driver License in accordance with the Agency Vehicle and Driving policy and transportation necessary to meet program needs.
