Critical Time Intervention Team Lead

Pay Rate: $70,000 - $77,000 Yearly
Schedule: Monday Friday: 8:00 a.m. 4:00 p.m.

Working closely with the Office for People with Developmental Disabilities and the Office of Mental Health, we support 27 housing locations and 3 administrative buildings.
About the Role:
We are looking for a proactive employee to use ones own experience and judgement to carry out the mission of the CTI model and Transitional Residential Services (TRS) for adults (18 ) boarding in hospitals and emergency rooms.
Responsibilities
- Ensure fidelity to Critical Time Intervention (CTI) case management model, including phase progression, contact frequency and associated documentation
- Offer clinical support and clinical recommendations, as needed
- Bridge any gap in clinical treatment, if necessary, by providing temporary clinical treatment until participant is linked with permanent mental health provider
- Carry caseload as needed, providing case management, in addition to clinical oversight
- Establish strong working relationships with local hospitals, and facilitate and support both the referral and intake process
- Ensure CTI staff meet with participants, families and other sources of support following discharge from hospital
- Review intake assessments to ensure identification of needs and establishment of realistic and appropriate goals
- Assist the CTI team, the participant, family and/or informal support system in determining additional services needed to achieve and maintain stability
- Responsible for the supervision of the CTI Care Manager
- Implement a system for ongoing team meetings and program CTI staff meetings
- Ensure that all required documentation is completed with required elements, and maintained, per Agency procedures/NYS regulations, including clinical documentation, progress notes, safety plans, intake/discharge plans, and clinical and non-clinical assessments
- Complete participant/family/support satisfaction surveys and analyze results
- Establish and maintain rapport with hospitals, participants, care managers, service providers, etc. to assist with linkages to services for the participants/family/supports
Education/Experience:
NYS Licensed Mental Health Professional, required (LMSW, LMHC, LMHC-D, LMFT). NYS Licensed Clinical Social Worker (LCSW), preferred. Three to five years of experience working with people with a mental health diagnosis, and/or intellectual/developmental disability (I/DD) dual diagnosis, preferred.
Conditions of Employment:
Must have a valid NYS Driver License in accordance with the Agency Vehicle and Driving policy and transportation necessary to meet program needs.
