Independent Housing Program Coordinator
Reports to: Program Manager MH IHP
Location: 6520 Basile Rowe, East Syracuse, 13057
Schedule: Monday Friday, 8:00 a.m. 4:00 p.m.
Pay Rate: $21-$23 Hourly

Our focus is supporting individuals by ways of residential housing, mental health and clinical services, and immersing our participants in community engagement. Working closely with the Office for People with Developmental Disabilities and the Office of Mental Health, we support 27 housing locations and 3 administrative buildings.
About the Role
We are looking for a proactive employee to perform duties related to participant screening, admission and caseload assignment within the Independent Housing Program in addition to direct support services consistent with supportive housing programming.
Responsibilities include:
- Develop supportive relationships with individuals and get to know their unique capabilities, personality and potential
- Respect the dignity, preferences, and individuality of all people
- Maintain a person-centered approach and support individual preference and choice to the extent possible under program guidelines
- Support participants by providing assistance with daily living skills and oversight to promote independence in the community
- Conduct program intake screenings and assessments to support rapid and efficient housing services to eligible participants
- Facilitate participants efforts to obtain and maintain safe, affordable housing
- Assist Program Manager in developing a variety of housing options that provide safe and affordable permanent housing to program participants
- Maintain relationships with community residence staff to support planning with individuals interested in exploring housing options
- Possess a broad knowledge of community services which can be shared with participants, family members, service providers, and other significant support people
- Assist in the referral processes to connect participants with relevant services; coordinate services with such programs to identify participants areas of need and delegate support appropriately
- Provide person-centered, quality services
- Follow Agency standards and procedures
- Ensure compliance with federal, state, and local employment laws and regulations, as well as company policie
- Perform Other Duties as Assigned
Education & Experience
Associates degree plus 2 years experience in human services, or, equivalent combination of relevant education and experience, required. Bachelors degree, preferred. Computer skills including word processing, Excel, and database management, required. Good written and verbal skills required.
Conditions of Employment
Must have a valid NYS Driver License in accordance with the Agency Vehicle and Driving policy and transportation necessary to meet program needs.
