CTI Case Manager, ESD

Pay Rate: $52,000-$59,000 Annually
Schedule: M-F: 8:00 a.m. 4:00 p.m.

Our focus is supporting individuals by ways of residential housing, mental health and clinical services, and immersing our participants in community engagement. Working closely with the Office for People with Developmental Disabilities and the Office of Mental Health, we support 27 housing locations and 3 administrative buildings.
About the Role:
We are looking for a proactive employee to use their own experience and judgement to carry out the mission of the CTI model and Transitional Residential Services (TRS) for adults (18 ), boarding in hospitals and emergency rooms.
Responsibilities:
- Meet with participants referred by partnering hospitals to the ESD program within 24 hours of referral
- Follow the CTI case management progression and maintain fidelity with the CTI model
- Complete intake assessments to determine needs, goals, and services needed to achieve and maintain stability in the community
- Engage individuals in skill building, offer crisis management, collaborate with other service providers, assist individuals in navigating complex systems and collaborate with informal supports in adherence to the CTI model
- Provide CTI case management for a caseload of assigned participants for 9 months
- Accompany assigned participants to meetings/intake appointments, as needed
- Observe participant progress; adjust supports, make referrals and changes, as needed
- Implement a warm hand off between the participant and other service providers
- Complete and/or update CTI documentation, including an intake assessment, Phase Plan and ongoing progress notes for each participant assigned
- Ensure that all required documentation is completed and maintained per Agency policy and procedure/NYS regulation
- Document contacts and referrals made for each assigned participant
- Ensure incidents and/or compliance issues are recorded and reported per Agency policy and procedure/NYS regulation
- Complete required reports as directed by supervisor, including quarterly progress reports on program goals
- Complete discharge plan summary for each assigned participant at the conclusion of CTI service provision
- Conduct self in a professional manner
- Attend staff meetings and other meetings as directed by supervisor
- Provide person-centered, quality, services
- Ensure compliance with federal, state, and local employment laws and regulations, as well as with company policies
- Perform Other Duties as Assigned
Education/Experience:
Bachelors degree in the human services field, required. One year of experience working with people with developmental disabilities and/or mental health diagnoses or case management experience working with adults, preferred. Position requires effective verbal and written communication skills. Must be able to demonstrate teamwork and professionalism. Basic word processing and e-mail skills required.
Conditions of Employment:
Must have a valid NYS Driver License in accordance with the Agency Vehicle and Driving policy and transportation necessary to meet program needs. Must respect confidentiality. Must be on-call for bona fide emergencies and after-hours program service needs.
