Duties
- Assist residents or clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and toileting to ensure comfort and dignity.
- Monitor patient health by observing vital signs, reporting changes, and maintaining accurate care plans aligned with healthcare standards.
- Support medication administration under supervision, ensuring proper documentation while adhering to HIPAA regulations to protect patient privacy.
- Facilitate safe patient transfers using manual handling techniques and equipment such as Hoyer lifts, ensuring safety for both clients and caregivers.
- Prepare nutritious meals tailored to individual dietary needs and assist with meal consumption when necessary.
- Maintain a clean, safe environment by performing light housekeeping tasks such as cleaning and disinfecting common areas and resident spaces.
- Provide transportation assistance for appointments or community outings, promoting social engagement and independence.
Skills
- Proven experience working with individuals with disabilities, including developmental disabilities, neurodevelopmental disorders like autism, or dementia-related conditions such as Alzheimer’s and other memory care needs.
- Strong knowledge of patient observation techniques, patient monitoring, and recognizing signs of distress or health deterioration.
- Familiarity with care plans development and implementation to ensure personalized support strategies are followed effectively.
- Competence in basic nursing skills including vital signs measurement, medication administration, first aid, CPR certification, and emergency response procedures.
- Ability to perform heavy lifting and manual handling safely while adhering to safety protocols to prevent injury.
- Excellent communication skills for engaging with clients respectfully while supporting their resident’s rights and fostering trustful relationships.
- Experience working within home health settings or long-term care environments such as nursing homes or hospice care is highly valued.
