Employment Type: Full-Time M-F (8am-4pm or 9am-5pm- 1 hr lunch break daily- 35 hours paid per week) *Bilingual in Spanish required*

Position Summary: Skills Trainer – Personal Care Management (PCM)

As a vital member of our interdisciplinary care team, the Skills Trainer plays a key role in supporting consumers enrolled in the Personal Care Management (PCM) program. This position includes responsibilities related to assessment, coordination, advocacy, instruction, and ongoing support to empower individuals in managing their own care and personal care attendants.

Key Responsibilities:

  • Provide intake and orientation for new PCM consumers, including eligibility determination, assessments, and initial training.
  • Educate consumers and/or surrogates on their roles, responsibilities, and rights within the PCA program using the PCA Program Manual.
  • Train consumers on hiring and managing PCAs, emergency management, and navigating fiscal intermediary processes.
  • Complete required documentation such as MassHealth PCA service applications and service agreements.
  • Conduct quarterly trainings for new consumers and upon request from stakeholders (MassHealth, Fiscal Intermediary, etc.).
  • Maintain compliance with PCM Quality Standards, including authorization accuracy, consumer satisfaction, and audit benchmarks.
  • Keep accurate and timely progress notes and consumer records using various data systems.
  • Participate in community outreach to identify eligible consumers.
  • Attend regular team meetings and contribute to program development.

Qualifications:

  • Associate’s degree in a relevant human services or social services field (related experience with some college coursework may be considered in lieu of a degree).
  • Bilingual in Spanish required
  • Experience working with individuals of all ages and abilities, including people with disabilities.
  • Strong communication, assessment, and training skills.
  • Basic computer proficiency (Microsoft Office, Excel, data entry platforms).
  • Knowledge of community resources and social/health service systems is a plus.
  • Ability to work independently while also collaborating within a team.
  • Must be organized, detail-oriented, and able to manage multiple priorities.

Additional Requirements:

  • Valid driver’s license REQUIRED and reliable transportation REQUIRED.
  • Satisfactory background checks including CORI, SORI, SAMs GSA, MassHealth Exclusion List, and OIG.
  • Ability to conduct intake and training services both in-person and via phone/video conferencing.

Pay: $20.00 - $22.00 per hour
