Registered Nurse – Case Manager
Location: Richmond, VA
Shift: Day, 4x10-hour (08:30 – 18:00)
Duration: 13 weeks
Position Overview:
We are seeking an experienced Registered Nurse – Case Manager to join a Level 1 facility. This role focuses on providing high-quality case management for patients, coordinating care, and ensuring optimal outcomes. The ideal candidate will have experience in acute care, strong clinical judgment, and excellent communication skills.
Key Responsibilities:
- Provide case management and care coordination for patients within the assigned unit.
- Act as a weekend float case manager as required.
- Collaborate with interdisciplinary teams to develop and implement patient care plans.
- Utilize clinical judgment to assess patient needs and make appropriate interventions.
- Ensure accurate and timely documentation using the required charting system (EPIC).
- Participate in quality improvement initiatives and follow facility protocols.
- Support patients, families, and caregivers by providing education and guidance on disease management and healthcare resources.
Requirements:
- Education: BSN required.
- Experience: Minimum 2 years of acute care experience; Level 1 facility experience required.
- Licensure: VA license or compact state license must be active and in hand.
- Skills:
- Case management expertise.
- EPIC charting experience required.
- Strong clinical assessment and decision-making skills.
- Other:
- Travel experience highly preferred.
- Candidates must be able to float within scope of practice if needed.
- Professional references required (minimum of 2).
- Compliance with facility policies including background checks and orientation modules.
Shift Details:
- 4x10 schedule, Thursday through Sunday.
- Weekend float responsibilities included.
- Orientation and required modules completed during initial start period.
