Job Summary
Under the supervision of the Director of Activities provides MDS documentation, Resident assessment of current interests and activity participation, and care planning.
Education & Experience
- Previous experience with the elderly
- Formal High School Diploma or equivalent
- Minimum of one year of Activities experience required, i,e. documenting Resident care planning, developing and implementing new activities.
Experience Desired
- Ability to play musical instrument
- Knowledge of art
- Knowledge of MDS
Skill Requirements
- Ability to lead and motivate groups of Residents
Essential Functions
- Documents the Residents initial assessment, interim assessments integrated progress notes, and Resident care
- Participates in team conferences and care planning
- Documents residents progress and Interaction during leisure time
- Develops Resident care plans tor Resident activity programs
- Directs and leads assigned activities; participates in off premise trips and events
- Modifies activities and/or equipment to facilitate participation in activities for Residents with disabilities
- Assists Residents on assigned units with activities of interest
- Guides volunteers who are assisting with activities on assigned units
- documents Resident attendance at activities programs
- Prepares, Instructs and completes crafts and other projects
- Encourages Residents to participate in activities appropriate to their needs, interests and physical and mental
- Submits requests for materials and supplies to the Director of Activities
- Ensures proper use of equipment, tools and supplies
- Transports Residents to and from activities programs
- Conducts recreational programs e. sensory/tactile stimulation, movement, art, music
- Ensures that animals are properly maintained for pet therapy
- Responsible for maintaining all plants indoors and outdoors, i,e. watering, trimming,
- Performs other related duties as required or directed.
