RN Case Manager

ABLE, Inc. is seeking a dedicated Registered Nurse Case Manager to provide comprehensive health care services to individuals with developmental disabilities. This role involves case management, health supports, staff training, and ensuring the well-being of individuals in our care.

Key Responsibilities:

• Case Management:

  
  

• Conduct comprehensive assessments and develop individualized care plans.

  
  

• Coordinate services and supports to address unique health needs.

  
  

• Monitor progress and revise plans as needed.

  
  

• Facilitate opportunities for growth and independence.

  
  

• Manage financial resources and coordinate with external agencies.

  
  

• Ensure compliance with applicable laws, regulations, and policies.

  
  

• Health Supports:

  
  

• Monitor physical and mental health needs, including medication management and appointments.

  
  

• Ensure adequate environmental support and assistive devices.

  
  

• Provide training and instructions to staff.

  
  

• Assist with hospital admissions, discharges, and transportation.

  
  

• Staff Training and Development:

  
  

• Provide training to direct care staff on nursing tasks.

  
  

• Ensure competency of staff in delegated nursing tasks.

  
  

Qualifications:

• Bachelor's Degree and Registered Nurse license in North Dakota

  
  

• Valid driver's license

  
  

• Proficiency in computer systems (Outlook, Teams, Therap)

  
  

• Experience with ICF/DD and The Council standards preferred
