Patient Retention Coordinator

Responsibilities
• Build and maintain positive relationships with existing patients and caregivers to promote satisfaction and ongoing services.
• Conduct follow-up calls to ensure patient needs are met and address any service-related concerns promptly.
• Collaborate with nursing, intake, authorizations, and scheduling teams to minimize service interruptions.
• Identify and report patterns that may impact patient retention or satisfaction.
• Participate in retention initiatives and campaigns to improve patient engagement and loyalty.
Requirements
• Excellent interpersonal, communication, and problem-solving skills.
• Empathetic, patient-focused, and proactive approach.
• Experience in customer service, healthcare coordination, or home care patient retention.
• Bilingual English/Spanish highly preferred.
• Ability to work in a busy office environment and perform detailed tasks using office equipment.
