scheduling. This role ensures seamless coordination of care, regulatory compliance, and effective communication among clients, caregivers, and internal teams.
Responsibilities
- Coordinate and manage client service schedules, ensuring full coverage and timely adjustments
- Act as the primary point of contact for clients and caregivers regarding service needs
- Onboard new clients, complete intake documentation, and develop care plans in collaboration with clinical staff
- Monitor ongoing cases to ensure quality care and client satisfaction
- Communicate regularly with aides to confirm attendance, address concerns, and provide support
- Ensure compliance with agency policies and state regulations
- Maintain accurate documentation and records in agency systems
Requirements
- 1 years of experience in a home care or healthcare coordination role
- Strong scheduling and organizational skills
- Excellent communication and interpersonal abilities
