We are seeking a motivated Nurse Practitioner or Physician Assistant to join our expanding Post-Acute Care team in North Conway, NH (additional openings in Wakefield, RI). This full-time role focuses on delivering high-quality, patient-centered care to individuals transitioning from acute hospital settings into skilled nursing and rehabilitation facilities.
You will collaborate with interdisciplinary teams to manage acute and chronic conditions, support recovery, reduce readmissions, and optimize patient outcomes through facility-based rounding. The position offers clinical autonomy with robust operational support and the chance to build long-term relationships with patients and care teams.
Work schedule and environment
- Full-time, Monday–Friday, 9:00 AM–5:00 PM
- No weekends, no on-call responsibilities
- Facility-based rounding in post-acute care settings

Compensation and benefits
- Base salary: $120,000 annually
- Total earning potential: $180,000–$220,000

Qualifications
- Graduate of an accredited NP or PA program
- Current, unrestricted NP or PA license in the applicable state, or eligibility for licensure
- National board certification (NP or PA)
- DEA registration or eligibility to obtain
- Minimum one year clinical experience preferred; experience in post-acute care, skilled nursing, geriatrics, internal medicine, family medicine, hospital medicine, or long-term care strongly preferred
- Strong clinical assessment, diagnostic, and care coordination skills
- Excellent communication and interpersonal abilities
- Ability to work independently while effectively collaborating within a multidisciplinary team
- Proficiency with electronic medical record (EMR) systems
