Provide home-based acute, post-acute, and transitional care to medically complex and recently discharged adult and geriatric patients across the San Fernando Valley and Antelope Valley. Operate as a field-based clinician with clinical autonomy while coordinating with an interdisciplinary team to improve outcomes and reduce readmissions.
Key responsibilities
- Conduct home visits and telehealth as needed for assessment, diagnosis, treatment, and care planning.
- Manage medically complex and recently discharged patients, including medication management, wound care, and coordination of services.
- Develop and implement individualized care plans and monitor patient progress.
- Communicate and collaborate with physicians, case managers, home health, and community resources.
- Document visits and clinical findings in the electronic medical record per organization standards.
- Participate in quality improvement initiatives and multidisciplinary case reviews.
Work setting and schedule
- Field-based role requiring flexible daily travel within assigned territories in the San Fernando and Antelope Valleys.
- Independent practice in patients’ homes with support from a multidisciplinary team.

Minimum qualifications and requirements
- Master of Science in Nursing.
- Adult/Geriatric Nurse Practitioner certification.
- Active, unrestricted California RN license and California NP license.
- Minimum 1 year of NP clinical experience.
- Strong assessment, diagnostic, and care-management skills.
- Valid driver’s license and reliable transportation.
- Ability to work independently and collaborate with a multidisciplinary team.
- Current TB test or willingness to obtain prior to employment.
