Provide field-based acute, post-acute, and transitional care to medically complex and recently discharged adult and geriatric patients in Orange County and LA County, CA. Work autonomously while collaborating with an interdisciplinary team to improve outcomes, reduce readmissions, and support safe recovery at home.
Key responsibilities
- Conduct home visits to assess, diagnose, and manage acute and chronic conditions for adult/geriatric patients
- Develop and implement individualized care plans, including medication management, wound care, symptom control, and care coordination
- Coordinate with physicians, specialists, home health, case managers, and community resources
- Document clinical findings and treatment plans in the electronic medical record in a timely manner
- Provide patient and caregiver education, advance care planning, and goals-of-care discussions
- Monitor patient progress, adjust treatment plans, and escalate care when necessary
- Participate in quality improvement and interdisciplinary case reviews

Minimum qualifications
- Master of Science in Nursing
- Adult/Geriatric Nurse Practitioner certification
- Active, unrestricted California RN and NP licenses
- Minimum 1 year of NP clinical experience
- Strong assessment, diagnostic, and care management skills
- Valid driver’s license and reliable transportation
- Current TB test or willingness to obtain prior to employment
Location: Orange County, CA (field-based; travel throughout assigned territories including LA County)
