Responsibilities
- Deliver comprehensive primary care across the lifespan, including diagnosis and management of acute and chronic conditions
- Provide preventive care, wellness exams, and routine health screenings
- Build long-term relationships with patients and families
- Coordinate care with specialists and the interdisciplinary team to optimize outcomes
- Apply evidence-based medicine and maintain accurate, timely documentation in the electronic medical record

Qualifications
- MD or DO from an accredited medical school
- Board Certified or Board Eligible in Family Medicine
- Completion of an accredited Family Medicine residency
- Eligible for or currently hold an unrestricted Indiana medical license
- DEA certification or eligibility to obtain
- Strong clinical, communication, and interpersonal skills
