A Day in the Life of a Community Connect Specialist
As a Community Connect Specialist, you serve as the face of our agency throughout the community. Your work helps ensure that individuals, families, and referral sources understand the services we provide and how we can support their needs.
Your responsibilities include:
- Making 50-60 in-person community visits per week (20-30 for part-time)
- Building relationships with churches, senior ministries, civic groups, attorneys, food banks, public health agencies, and other community partners
- Conducting quality assurance visits with clients, families, and referral sources
- Educating the community about Medicare guidelines and available home health services
- Tailoring presentations and educational materials to meet the needs of each audience
- Participating in community events, health fairs, and public speaking engagements
- Researching and implementing new programs to enhance service quality and communication
- Staying within a $500 monthly budget (with approval required for overages)
- Submitting weekly Community Education Pre-Plans and quarterly action plans
Your work strengthens our presence in the community and ensures that individuals know where to turn when they need compassionate home health care.
Qualifications
- Degree in marketing, health, or a related field OR licensure as a medical professional
- At least 1 year of experience in healthcare marketing, public relations, or clinical practice
- Knowledge of agency service lines and disease specialty programs
- Willingness to participate in community events and health fairs

Work Schedule
Full-time, Monday-Friday
8:00 AM - 5:00 PM
