Job Title:  Scheduler II

Job Location: Concord, CA 94520 (Hybrid)

Job Summary:

We are seeking a Scheduler to support Home Health operations in Concord. This role is responsible for maintaining patient service schedules, coordinating with physicians' offices and internal departments, updating documentation, and ensuring timely communication with patients and care teams. This is a non-patient-care, clerical scheduling position requiring strong organizational skills and the ability to manage multiple priorities in a fast-paced environment.

Responsibilities

  • Maintain accurate patient service schedules and related documentation.
  • Coordinate scheduling activities with physicians’ offices, patients, families, and internal departments.
  • Answer phones and communicate scheduling updates to care teams.
  • Confirm availability and resolve routine scheduling issues.
  • Support branch coverage during leaves of absence and staffing needs.
  • Ensure compliance with privacy regulations and protection of PHI.
  • Perform administrative and clerical duties while managing multiple tasks effectively.

Required Qualifications

  • High School diploma or equivalent education/experience.
  • Minimum 1 year of recent scheduling, clerical, or healthcare administrative experience.
  • Experience with patient scheduling and documentation updates.
  • Knowledge of medical terminology, diagnostic tests, surgeries, consultations, and procedures.
  • Proficiency with Microsoft Office Suite (Word, Excel, Outlook).
  • Ability to work independently and prioritize tasks in a fast-paced environment.
  • Strong attention to detail and commitment to confidentiality and regulatory compliance.

Preferred Qualifications

  • Home health or centralized scheduling experience.
  • Experience supporting high-volume branch operations or patient census workflows.
  • Strong written and verbal communication skills.
  • Ability to build effective working relationships with peers, supervisors, and care teams.
