TITLE: Program Supervisor

STATUS: Full-Time, Regular

CLASSIFICATION: Hourly, Non-Exempt

OFFICE HOURS: Monday-Friday; 9am-530pm - occasional overtime occasional weekends

POSITIONS SUPERVISED: CSFs (Case Managers), House Managers, Mental Health Providers, Community Living Instructors

REPORTS TO: Program Director

Under supervision of the Director, helps to promote the mission of the A Bright Future, Inc by coordinating the efforts of staff to support and train individuals with developmental disabilities in a variety of community-based settings as part of the A Bright Future Program Design. This position provides direct support to individuals served as well as provides coordination for program activities.

Essential Tasks

Coordinate Services

Ensure that persons served have access to community services/resources, opportunities to explore educational, social, recreational and employment goals and the chance to make/maintain connections with others. Includes daily mobility training to assist persons served to access community settings through use of public and other forms of transportation.

  • Coordinate the array of services needed to achieve each person’s individual goals and objectives, including: use of assistive technology, job analysis, task analysis, assessment, systematic instruction; and ongoing support services as is necessary
  • Ensure that staff assist program participants to develop the tools necessary (i.e., resume, master application, DMV ID, etc.) to achieve goals such as obtaining employment, accessing preferred environments, and making connections with others
  • Provides other types of support and instruction to persons served as requested

Develop/Implement Service Plans and Write Reports

  • In coordination with the ID team, facilitates a person-centered planning process and works with staff to develop Individual Service Plans (ISP’s) based on the person’s skills, abilities, and interests. Facilitates ISP meetings for program participants
  • Coordinates staff activities to ensure that the A Bright Future Program adheres to A Bright Future, Inc and funding/regulatory agency expectations regarding data collection, progress reporting, and the submission of reports
  • Maintains files for persons served in accordance with A Bright Future, Inc. policies
  • Reports all suspected abuse in accordance with mandated reporter regulations and procedures and adheres to A Bright Future, Inc expectations for reporting special incidents
  • Adheres to all related agency policies and procedures

Create Community Connections

  • In coordination with other A Bright Future, Inc team members, seeks opportunities to work collaboratively with employers and community/service organizations. This includes actively seeking employment and other inclusive opportunities for persons served, following up on leads from other team members and maintaining connections as requested
  • In coordination with persons served, facilitates and assists individuals to develop a circle of support that builds and maintains connections with family, friends, neighbors, and community members
  • Provides education and support to community members and assists individuals to negotiate support needs

Develop and Maintain Effective Relationships

  • Displays a professional and respectful attitude towards individuals served, coworkers, supervisors, community employers and other agency affiliates
  • Works effectively as part of the A Bright Future, Inc team to accomplish the mission of the organization. This includes developing and maintaining positive relations with families, other agencies serving individuals with disabilities, funding sources/other government agencies and all entities that provide services which overlap with programs provided by A Bright Future Academy
  • Models person centered thinking philosophies in all work efforts and consistently promotes persons served to be at the center of making decisions that impact their own lives
  • Contributes to a team approach to solving issues related to persons served
  • Provides for continuous quality service improvement and promotes teamwork among co-workers and persons served

Prerequisite Qualifications

The candidate must meet the following criteria to be considered for employment in this position:

  • 21 years of age or older
  • High School diploma or equivalent required. Bachelor’s degree in social services preferred
  • Three (3) years’ experience working in a human services related field; and the demonstrated ability to provide staff training, supervision and planning
  • One (1) year minimum experience coordinating services for individuals with disabilities preferred
  • Experience in job/task analysis, work assessment, teaching techniques or equivalent experience is desirable
  • Valid CA driver’s license, acceptable driving record and current auto insurance for a car that will be used during work time as needed
  • Prior to employment, A fingerprint/ Criminal record clearance with a Child Abuse Index check from the Department of Justice,
  • Prior to employment, current physical examination and tuberculosis test
  • Ability to work a flexible schedule which may include nights and weekends
  • Ability to self-direct daily work activities with little supervision
  • Ability to maintain an orderly work environment, prioritize workload and perform tasks in a prescribed and safe manner
  • Demonstrates sensitivity to the multi-cultural diversity of the service population
  • Willingness to enroll in ongoing, free training to expand and develop skills
  • Experience with Windows operating system, Microsoft Office and database reporting
  • 1st Aid and CPR certifications are required
