Job Title: Clifton Place Case Manager (Onsite)

Sign on Bonus: Sign on Bonus of $600 (Half paid out at 3 months and the other half at 12 months).  

Salary Range: $24.04 – 26.44 per hour; $50,000.00 – $55,000.00 annually

 Description

The Case Manager provides intensive, on-going case management services and supervision of an assigned caseload of adult clients within a residential, sober, supportive housing site. Skilled in trauma-informed care, Case Managers help adults who are involved in the justice system work to secure community resources, such as employment, services, and documentation. 

This position does not have direct reports. This position is 40 hours per week, onsite.

Duties and Responsibilities include the following. Other duties may be assigned.

Intake and Onboarding

  • Meet with potential clients inside correctional facilities for orientation as needed.  
  • Complete intake process and make appropriate referrals within 24 to 48 hours. 

 Case Management and Documentation

  • Assist clients with eligibility for GRH, medical, and county benefits. 
  • Knowledgeable about the housing market for clients with criminal backgrounds. 
  • Coordinate with clients to meet their employment, housing, mental health, CD and cognitive needs with other professionals in the community. 
  • Communicate with client’s professional contacts including but not limited to Correctional professionals, Mental Health, Chemical Health, and Medical initially and on-going. 
  • Create comprehensive case plans at entry to the program, update every 30-days and upon exit.  
  • Keeps updated Individual Case Plans, Case notes, and any other programmatic forms as directed by the Senior Manager. 
  • Submit bi-weekly progress reports to correctional professionals.  
  • Manage and provide direct services to clients to monitor compliance with court ordered conditions of release and remain law abiding. 
  • Have at least one face to face contact with each client assigned to caseload a minimum 1x week. 
  •  Create with clients, sign and monitor client’s daily planners for accountability purposes. 
  • Monitor client’s progress in maintaining health and wellness, income, housing, employment opportunities, and individualized topics such as chemical dependency programming, psychological/mental health resources, and accessing appropriate benefits opportunities. 
  • Document client contact and service coordination accurately and timely. 
  • Transport or arrange transportation for clients to medical, court, or other appointments. 
  • Attend community meetings relevant to program and clients. 
  • Maintain knowledge of the Department of Correction system, relevant laws, regulations and policies regarding reporting requirements, data practices, and client rights. 
  • Monitor/maintain public and facility safety within job assignments, includes monitoring safety and security of 180 if assigned. 
  • Maintain flexibility to work in a residential 24-hour facility including evenings, holidays, weekends, and overnights as needed to meet the needs of clients and the agency. 

 Cessation of Services

  • Complete termination and discharge reports to be submitted to correctional professionals within 48 hours. 
  • Facilitate staffing with clients in a timely manner who experience violations and face termination with community collaterals and PO’s.

 Provide successful coordination of services for a smooth entry into the facility and a supportive discharge out of the facility.

  • Work with clients to ensure transition plans are effective and supportive.
  • Ensures clients are compliant with court orders and remain law abiding. 
  • Monitors client progress as including housing, employment, and community progress.  
  • Identifies resident needs and makes appropriate referrals to other county or community agencies; meets with residents frequently.
  • Creates comprehensive case plans and transition plans that reflect the needs of residents. 
  • Documents resident contact and service coordination accurately and timely. 
  • Works effectively and collaboratively with other professionals in the field. 
  • Arranges transportation for clients to medical, court, or other appointments. 
  • Maintains knowledge of culturally specific services available in the community. 
  • Is current with the justice system, relevant laws, regulations, and policies regarding social services provision, reporting requirements, data practices, and resident rights.

 Ensure clients are provided excellent service consistent with evidence-based practices in order to set client-specific goals and evaluate outcomes 

  • Ensures the intake process is effective with accurate data and documentation.
  • Maintains awareness of resident activities in and away from the facility 
  • Uses knowledge of alternative systems to manage or deescalate potential situations. 
  • Utilize trauma informed techniques when working with staff and clients 
  • Ensure compliance with all regulatory and licensing standards 

 Actively contribute to Clifton Place.

  • Demonstrate effective teamwork and collaboration with management and colleagues. 
  • Ensure all services align with best practices for effective client support. 
  • Engage in the preparation and attendance of case consultations, ensuring inclusion of relevant community partners and facility staff while respecting data privacy.
  • Active participant as member of the facility team, including potential on-call rotation and potential programming facilitation. 

 Qualifications: 

  • Minimum age: 21.
  • Bachelor’s degree in social work, social services, psychology, criminal justice, or a related field and two years of case management experience related criminal justice or community re-entry; relevant work experience may be substituted in lieu of education on a year-for-year basis.
  • Experience in the nonprofit sector preferred.   
  • Valid driver’s license and insurance; dependable transportation for traveling to community partners and events.
  • Ability to clear a Department of Human Services background screening.

Non-Profit Location/Work Environment

This is an onsite role based at a 180 Degrees residential property in Minneapolis. The environment is fast-paced and requires flexibility, attention to safety, and regular interaction with residents. The employee may be required to work extended or irregular hours to support staffing, events, emergencies, or critical projects.
