Building Location: Building A - Duluth Clinic - 1st Street Department: 1008370 CHNA - EH SS Job Description: CHWs are natural helpers and individuals who have a close understanding of the communities they serve. They are specifically hired and trained to address behavioral and social determinants of health through patient-centered goal setting. CHWs reach beyond clinic walls to help patients meet their goals. They creatively leverage patient, family, and community resources to address root causes of patient’s health concerns. CHWs work to increase access to medical, behavioral health, and community services. The goal of this team is to improve the patient’s overall wellbeing through developing trusted relationships with the patient and their family. Education Qualifications: Key Responsibilities: Outreach and enroll eligible patients Build trusting relationships with patients and work together to create personalized goals to improve wellbeing Provide tailored support in accessing health care, community connection, and social support Provide referrals for social service resources in the community Advocate for needs of patients and offer social support Communicate with healthcare team about cultural concerns and barriers faced by the patients in the community Required Qualifications: At least 1 year of experience in customer service, social service, healthcare behavioral health or related field Preferred Qualifications: Strong written and verbal communication skills Knowledge of local community resources A desire to make a healthy difference in people’s lives Licensure/Certification Qualifications: Required Certification: Community Health Worker certificate or ability to achieve within 18 months of employment or date of hire FTE: 1 Possible Remote/Hybrid Option: Shift Rotation: Day Rotation (United States of America) Shift Start Time: 8:00 AM Shift End Time: 4:30 PM Weekends: Holidays: No Call Obligation: No Union: Union Posting Deadline: Compensation Range: $19.08 - $28.62 Employee Benefits at Essentia Health: At Essentia Health, we’re committed to supporting your well-being, growth, and work-life balance.
