JOB DESCRIPTION
POSITION: Community Health Worker
LOCATION: Red Cliff Community Health Center
WAGE: $18.00 – 21.00 per hour
SUPERVISOR: Community Health Supervisor
THIS IS A REGULAR PART-TIME NON-EXEMPT POSITION
JOB SUMMARY: The Community Health Worker helps individuals and families to improve their health, increase their access to health and social services, and reduce health inequities.
DUTIES AND RESPONSIBILITIES:
- Participate in community health outreach activities:
- Advise clients or groups on issues related to self-care, such as diabetes management.
- Identify high risk members of target population with overdue health needs.
- Distribute culturally appropriate educational materials such as flyers, brochures, or other material to inform members of the target population.
- Attend community events to engage and strengthen relationship between community and health service system.
- Transport or accompany clients to scheduled health appointments or referral sites.
- Provide feedback to health service providers regarding improving service accessibility.
- Provide case management type services including periodic and on-going assessments, referrals to other resources and assistance with overcoming barriers to access.
- Participate as a member of the Community Health, respiratory season response team which may include testing, education, and support.
- Collaborate with other service programs to assist clients with applying for social services, including Medicaid, Long Term Care, Foodshare etc.
- Maintain strict confidentiality and safeguard privacy of patients in common areas.
- Advance job knowledge and skills through continuing education efforts with the approval of supervisor and/or Health Center Administration.
- Attend staff and other meetings, in-services, and other events as directed by supervisor.
- Maintain a clean and safe physical environment. Alert administration and/or supervisor to problems and difficulties, as circumstances may warrant.

KNOWLEDGE:
- Organization skills and attention to detail.
- Computer skills; data entry, Excel program, Word, Publisher.
QUALIFICATIONS:
- Minimum of high school diploma, HSED or GED required.
- Minimum of Certified Nursing Assistant certification or completion of Community Health Worker Program via Area Health Education Careers (AHEC) required.
- Three years’ experience working in a clinical setting – preferred.
- Current CPR certification or willing to accomplish within 90 days of employment.
- This position must possess and maintain a current valid Driver License. Must be eligible to be placed on the tribe’s vehicle insurance.
PERSONAL CONTACTS: Considerable daily contact will be made with the general public, tribal leadership, Program Directors and staff, consultants, and the Tribal Community.
PHYSICAL REQUIREMENTS: The duties assigned to this position involve bending, stooping, lifting, and carrying. Items may be placed in overhead storage or shelving.
WORK ENVIRONMENT: Red Cliff Community Health Center; office and clinic settings. Exposure to hazards of the health care industry. All Tribal Buildings are smoke free.
TRAVEL REQUIREMENTS: The ability to travel and attend meetings and trainings; overnight and out of town, both locally & nationally.
