POSITION DESCRIPTION:
This position falls within the broader scope of duties of a Service & Support Administrator which includes facilitating, coordinating, and monitoring services for children and adults with intellectual or developmental disabilities. The focus of this role is to provide support to, and act as a liaison between all necessary departments to support individuals in community-integrated living, increased independence, and progress on their path to inclusion.
As an employee of the Miami County Board of Developmental Disabilities (Riverside), the job incumbent shall always comply with all Board policies, and shall demonstrate respect for, support the dignity of, and observe the rights of all individuals served by the Board and demonstrate sensitivity to diverse beliefs, cultures, values and family structures.
QUALIFICATIONS:
· Bachelor’s Degree in Human Services, Education, Social Work, Rehabilitation or closely related field
· Two (2) years of experience in developmental disabilities or other related social service position
· Possession of or ability to obtain and maintain Service & Support Administration registration and/or certification from the Ohio Department of Developmental Disabilities within the probationary period
· Must support the Person-Centered Philosophy
· Strong oral and written communication
· Competency in Microsoft Office applications
· Ability to abide by Riverside’s Code of Conduct
· Satisfactory criminal background and Abuser Registry checks
· Satisfactory pre-employment drug test
· Possess valid State of Ohio driver’s license
DUTIES:
· Assists with Riverside’s provider support, recruitment, and retention efforts. Collaborates with providers to strengthen provider relationships, supports provider efforts in recruitment and retention of direct support professionals (DSPs) and other provider careers.
· Assists providers in completing the initial certification process, meeting their service provision rule requirements, and maintaining their certification. Assists providers with understanding and navigating service provision documentation, Medicaid waiver billing systems, and Electronic Visit Verification systems.
· Links individuals served in finding providers for Home Health, HPC, Adult Day, and other waiver services. Completes provider interviews or connects providers to individuals.
· Coordinates Miami County’s participation in the Rental Assistance Program (RAP) through DODD.
· Coordinates the treatment of household pests to support individuals served and providers to maintain community integrated housing.
· Identifies, develops relationships, and maintains a list of community resources to meet the physical, medical, and mental health needs of individuals served.
· Assist individuals served with applying for and maintaining public benefits and exploring other funding available.
· Conducts Pre-Admission Screen and Resident Review (PAS/RR) evaluations for DODD and nursing facilities. Conducts meetings with individuals at nursing facilities and assesses current and future needs.
· Must meet requirements to provide targeted case management (TCM) services to or on behalf of a Medicaid eligible individual
· Performs other related duties as may be assigned
HOURS: Monday – Friday 8:00 a.m. – 4:00 p.m.
SALARY: $21.52/hour or higher based on experience
FLSA: Non-Exempt from overtime
