**Home Care LVN**

at WelbeHealth Pasadena, CA Reporting to the Home Health Manager , the Home Care LVN is responsible for carrying out tasks in the participants residences as assigned such as home safety assessments, data gathering and supervisory visits**. A set schedule that encourages work/life balance for our clinical staff! This position is Monday to Friday, 8am to 4:30pm.** * PACE is a truly special model that allows practitioners to provide care in in a compassionate and relationship-focused way. **Your** **qualifications** **:** Minimum of two years experience in a clinical setting with a frail or elderly population Highly motivated, self-directed, able to execute tasks in a quickly changing environment and make sound decisions in emergency situations Excellent clinical, organizational and communication skills in settings with seniors, their families and interdisciplinary team members Experience and competency working with people from diverse backgrounds
