ABOUT VNA & HOSPICE OF THE SOUTHWEST REGION

Education

  • Master's Degree in Social Work preferred.

Experience

  • Two years in human services and/or social work experience in a medical environment required.
  • Prior experience in home care preferred.
  • Valid Driver's License

Responsibilities include but are not limited to:

  • Checks daily referrals/communication notes and schedules client visits.
  • Effectively integrates new and existing client schedules to maximize productivity in accordance with physician's orders. Effectively analyzes visit patterns and makes adjustments to scheduled visits accordingly.
  • Assesses and evaluates patient and family needs in order to provide quality care to assigned patients.
  • Develops, discusses and communicates a realistic plan of care for each client, including caregiver in order to address all identified needs.
  • Implements social service plan of care and procedures as necessary to ensure the safe effective quality of care for clients.
  • Evaluates a variety of aspects of implemented plan of care including, but not limited to identified problems, care provided and client's responses to meet client and family needs and ensure overall quality of care delivered.
  • Maintains up-to-date and accurate documentation of social services provided to ensure the effective integration of information for use by the health care team to ensure on-going and continued quality of care.
  • Coordinates a wide variety of ancillary client care services with physicians, providers, team members, etc. in support of the recommended plan of care to ensure safe and effective client and family care.
  • Proactively engages in CQI efforts to ensure continual professional growth.
  • Pro-actively advocates for client care issues to ensure that overall quality and type of care is sensitive to each specific patient/family's needs.
  • Performs client and care giver education to ensure informed patient decisions can be made, including, but not limited to, effectively executing Vermont Ethics Network advanced directives, treatment options, discharge instructions, reportable conditions, ranges of services, .
  • Attends and schedules case conference/family meetings with other team members to ensure collaboration of patient care.
