A DAY IN THE LIFE OF A HOSPICE QUALITY IMPROVEMENT SPECIALIST

The Quality Clinical Educator is responsible for overseeing the orientation of and ongoing training of clinical staff Agency Wide to ensure the delivery of safe, high-quality care in an efficient and cost effective manner to patients in a home setting. Responsibilities include but are not limited to the following:

  • Oversees clinical preceptor program and assures clinical competency checklists are current.
  • Provides one-on-one orientation and instruction to new clinicians in collaboration with the Clinical Manager.
  • Serves as an in-house consultant to assist clinical teams.
  • Assures technical competency by assessing learning needs, coordinating learning opportunities and evaluating the effect of training and education on practice.
  • Maintains a comprehensive understanding of the Medicare Conditions of Participation as it relates to Home Care and/or Hospice regulations.
  • Effectively designs learning experiences and patient care assignments for new staff and current staff as necessary.
  • Assesses new staff's readiness for patient care assignments, for quality of care related to best practices, such as PDGM episode management, utilizing tele monitoring, chronic disease (CHF, COPD), visit protocols, etc..
  • Performs home visits with clinical staff as requested by preceptors or Clinical Manager to educate on clinical practice and/or documentation.
  • Provides clinicians with the educational tools necessary to carry out their job responsibilities; i.e. Agency procedures, protocols, patient educational materials, in-service opportunities.
  • Collaborates with Chief Clinical Operations Officer and the Clinical Managers in relation to ongoing staff education and training programs.
  • Performs chart audits and provides education and/or recommendations based on trending to Clinical Managers and preceptors for chart documentation improvements.
  • Organizes, schedules, and manages on-going clinical education/in-services to include, but not be limited to, in-service calendars, LNA in-services.
  • Communicates with a variety of educational institutions, professional associations, consultants, and other external organizations in trouble-shooting clinical information systems as well as coordinating continuing education and training programs as needed.
  • Coordinates staff education and implementation of quality initiatives.

QUALIFICATIONS FOR HOSPICE QUALITY IMPROVEMENT SPECIALIST

  • Registered Nurse with a minimum of Bachelor's degree required, Master's preferred.
  • Minimum of two years clinical acute care nursing experience preferred with home care experience and/or previous case management experience in a home care setting required.
  • Experience teaching adults in a clinical setting preferred.
  • Valid state driver's license required.
