ESSENTIAL JOB DUTIES
•ssists with discharge planning primarily in Acute Care.
•Work with IDT to identify needs.
•Discuss/arranges options, services, and resources to help with areas of need.
•ssist with Health Care Power of Attorney documents and other Advanced Directives as able.
•Complete needed, required, and professional documentation and paperwork for documentation of patients' charts and other jobs tasks and requirements.
•Perform other duties as requested.
