Position Summary

The Housing Case Manager supports families experiencing homelessness by assisting them to rapidly access permanent housing and supportive services to achieve long-term housing stability. The position provides case management services to address multiple complex needs and reduce barriers, including referrals to community resources and linkages to other supportive services such as child care, employment services, health and wellness services, life skills workshops, and aftercare services to support stability. This position will work in our Interim Housing facility located in Santa Monica.

ESSENTIAL RESPONSIBILITIES:

  • Complete intake and assessment on all participants identified to determine most appropriate level of housing intervention.
  • Identify and assist participants in securing housing, basic needs, medical and mental health services, substance abuse treatment, and public benefits where eligible; develop a Housing Stability or Individual Service Plan in conjunction with the participant.
  • Provide strengths-based case management and service coordination designed to assist participants in obtaining and maintaining stable housing.
  • Meet with participants on a weekly basis (more often when needed), to monitor progress and /or barriers toward achieving their Housing Stability or Individual Service Plans.
  • Utilize evidence-based practices in service delivery such as Trauma-Informed Care, Intensive Case Management, Motivational Interviewing, Harm Reduction, and Housing First practices.
  • Provide crisis intervention services as needed.
  • Maintain accurate and timely records of activities, case management notes, and services provided to each client using HMIS and any other applicable data management system.
  • Develop and maintain relationships with private, public, and nonprofit landlords, including municipal housing departments, in order to access housing opportunities.
  • Perform site visits of prospective units to ensure they meet guidelines for habitability standards.
  • Ensure subsidized units are within the Fair Market Rate (FMR) guidelines; negotiate rent when necessary.
  • Provide advocacy as needed to help address issues and barriers between landlord and participant whenever applicable.
  • Develop and maintain relationships with participants, staff and local service providers.
  • Attend staff and other community meetings as necessary; and serve as a representative of UBH at community events.
  • Prepare and present assigned cases at weekly/monthly Participant Progress Review supervision meetings.
  • Provide regular, written reports to the Program Manager and Management as assigned.
  • Other duties as assigned.

 QUALIFICATIONS:

  • Bachelor’s Degree, from an accredited College or University, in Human Services (e.g., psychology, social work, sociology or related field) required.
  • Minimum of two-years experience providing services to persons experiencing homelessness, preferably families with minor children.
  • Exhibit high level of professionalism.
  • Demonstrated experience in working with people experiencing behavioral health issues, such as mental health, substance use, sex trafficking, or special education.
  • Experience working with a diverse population, with sensitivity to differences in culture, religion, sexual orientation, race, age, and physical and mental illness.
  • Understanding of best practice approaches including: Housing First, Motivational Interviewing, Harm Reduction, and Trauma-Informed Care.
  • Knowledge of community resources.
  • Strong written and verbal communication skills.
  • Ability to maintain personal and professional boundaries
