Pay $16.00 - 17.50 hour
The Community Habilitation Specialist is responsible for the development, implementation, and monitoring of individualized family supports and staff action plans, in compliance with state, federal, and agency regulations and operating standards.
Core Responsibilities
* Completing initial waiver service intakes with people we support and their families.
* Identifying individual habilitation needs and family support needs.
* Develop and monitor individual Staff Action Plans for people receiving services.
* Promote independence in the people we support by developing person centered programming.
* Respect the privacy and home environment of the people we support.
* Coordinate facilitation of group activities within the waiver department.
* Delivery of habilitation services to individuals while assessing appropriateness of plan.
* Attend to the person's needs and implement services as outlined in their Staff Action Plan.
* Complete weekly/monthly quality assurance review of staff's documentation and follow up as needed.
* Complete annual and semi-annual reviews of the people we support service plans and participate as a member of their planning team.
* Supervision of direct support staff; including performance, initial and on-going training; and coordination of staffing approved hours.
* Maintain active charts for the people we support; ensuring all documentation is current and meets regulatory requirements.
* Participate in leadership activities as part of the Community Services team; including meetings, trainings, and committee involvement.
* Ensure positive and effective communication within and between department, division, agency, family and community personnel.
* Ensure Compliance with all pertinent government and agency requirements.
Qualifications
* Associate's Degree in human services or related field (I.e. psychology, sociology, etc.)
* Minimum of 12 months experience in working with people with developmental disabilities.
* A minimum of 18 hours per week of direct service delivery will be required to be provided.
* Must have valid NYS driver's license and reliable transportation
