Community Nursing Services (CNS) is Hiring! Join our team as a PRN or Part-Time Registered Nurse Case Manager!

Position Details:
- Schedule: Monday – Friday
- Hours: 8 am to 5 pm
- Weekends: On-call once a month or every other month

Responsibilities:
- Act as a vital member of the Multidisciplinary Group.
- Provide compassionate home health and hospice services to individuals and their families.
- Manage care for a caseload of primarily complex clients under general direction.
- Offer clinical supervision to staff Registered Nurses.
- Assume a leadership role by providing supervision to CNA's associated with the caseload.
- Collaborate with the multidisciplinary group and other caregivers to achieve specific goals.
- Ensure access to other disciplines, as needed.
Requirements:
- Current RN licensure in the State of Utah.
- Possession of a current Utah driver’s license and reliable transportation.
- Excellent communication skills.
- Experience with in-home patient visits, palliative care, medicine management, wound care, and IV skills.
