Community Nursing Services (CNS) is Hiring! Join our team as a PRN or Part-Time Registered Nurse Case Manager!

Position Details:

  • Schedule: Monday – Friday
  • Hours: 8 am to 5 pm
  • Weekends: On-call once a month or every other month

Responsibilities:

  • Act as a vital member of the Multidisciplinary Group.
  • Provide compassionate home health and hospice services to individuals and their families.
  • Manage care for a caseload of primarily complex clients under general direction.
  • Offer clinical supervision to staff Registered Nurses.
  • Assume a leadership role by providing supervision to CNA's associated with the caseload.
  • Collaborate with the multidisciplinary group and other caregivers to achieve specific goals.
  • Ensure access to other disciplines, as needed.

Requirements:

  • Current RN licensure in the State of Utah.
  • Possession of a current Utah driver’s license and reliable transportation.
  • Excellent communication skills.
  • Experience with in-home patient visits, palliative care, medicine management, wound care, and IV skills.
