Job Title:

Nurse Practitioner - SOM Medicine Adult Palliative

Job Summary:

To provide palliative care and consultations as a palliative and support care team member in the acute inpatient care setting; function as a member of the interdisciplinary team to provide palliative services to patients with serious illness and their families; serve as liaison to community based hospice.

Education & Experience

Graduate of an approved nurse practitioner program and hold national certification. Must have Mississippi APRN license or eligible Mississippi APRN license. Must apply for and be awarded clinical privileges through University Hospitals and Health System Medical Staff before hire date.

CERTIFICATIONS, LICENSES OR REGISTRATION REQUIRED: MMississippi APRN license or eligible for Mississippi APRN license, must obtain nurse practitioner certification from the Mississippi board of nursing prior to hire date.

Knowledge, Skills & Abilities

Knowledge of nursing care procedures and protocols. Excellent verbal and written communication skills. Interpersonal skills. Knowledge and understanding of computer systems, tools and programs. At least 2 years of experience as advanced practice nurse and 2 years of hospice and palliative care experience preferred.

RESPONSIBILITIES

* Provides palliative care, consultations and supportive care to min acute inpatient care setting to meet the needs of the patient as requested by the referring physician

* Assesses psychosocial needs of the patient and family

* Provides recommendations for medications and other therapeutic forms of treatment in an effort to mitigate symptoms related to serious illness

* Documents visits/discussions in epic

* Communicates findings/recommendations to care team

* Participates in discharge planning process to assure safe transitions across the care continuum

* Remains current on issues involving goals of care conversations, advance health care directives

* Participates in staff and community education related to palliative care issues and end of life care.

* Participates in weekly palliative care team meeting to review active cases

* Serves as educator and mentor to graduate nursing students and new hire advance practice providers

* Functions as an expert/resource to nursing staff in area of specialty

* Serves on at least one nursing council. Hospital or medical center committee, task force, or project team to improve patient care or work area. Demonstrates core standards of empathic patient care.

(occasional-up to 20%, frequent-from 21% to 50%, constant-51% or more)
