Community Healthlink (CHL) recognizes the power of a diverse community and seeks applications from individuals with varied experiences, perspectives, and backgrounds.Community Healthlink (CHL) is looking for a Community Crisis Stablization (CCS) LPN in Worcester.

The North CountyCrisis Stabilization Unit (CSU)serves clients who are admitted due to struggle with a mental health crisis often additionally struggling with substance abuse. Our clients enter the program with an evaluation and client history completed by a qualified clinician prior to admission. The CSU may be used as adiversion to prevent a hospitalization or as a step-downimmediately following a psychiatric hospital discharge to assist the client in transitioning back into the community. The unit receives admissions 24 hours/day, 365 days/year depending on bed availability (13 beds) for clients who do not need hospital level of care and meet the unit's admission criteria. Although a voluntary program, individuals must remain on the unit for the duration of their stay for safety and treatment purposes. Clients meet with a multi-disciplinary team to aid with following their individualized treatment plan and meet their treatment goals as well as a psychiatric workup and medication evaluation.

Essential Responsibilities:

  • Assesses the physical and psychosocial status of patients in all settings by obtaining appropriate patient histories, administering physician/primary care provider orders, nursing assessment, medication administration reconciliation, takes and monitors vitals of clients on the unit, medication administration and documentation.
  • Provides appropriate health care and coordinates referrals as prescribed by providers.
  • Promotes education and adherence to prescribed treatment protocols.
  • Communicates and collaborates with health care facilities when clients are admitted and plans for discharge to ensure continuity of care.
  • Facilitates wellness education to individuals and offer groups with health and wellness focus.
  • Records all client information within electronic medical record system daily, received by outside healthcare agencies.
  • Administers treatments and medications per provider order.
  • Performs daily quality control checks for the practice.
  • Offers nursing consult on emergency service evaluations.
  • Assists with population health screenings and outcomes.
  • Demonstrates person centered strength-based communications and relationship with consumers using recovery-oriented principles.
  • Maintains and updates knowledge in accordance with the current trends of nursing and compliance with all standards of agency policies and procedures.
  • Demonstrates proficiency in completing electronic documentation in compliance with grant requirements, agency, state, and federal guidelines and policies.
  • Performs other related duties.

  • Must be able to move about the space periodically during the shift.
  • Must be able to stoop, crouch, kneel or crawl for various reasons.

Qualifications:

  • Graduate of an accredited school of Nursing with current license as a Licensed Practical Nurse in the state of Massachusetts.
  • 2-4 years' experience in medical nursing essential.
  • Recent experience in mental health/substance use preferred.
