Responsibilities
Rockford Center is seeking a dynamic and talented Admissions Nurse for Per diem with varied 12hr shifts available!
Rockford Center is a 138-bed mental health facility located in Newark, DE, providing easy access to a full complement of inpatient and partial behavioral health programs for children and adolescents , adults , and older adults . Rockford is the only child inpatient behavioral health facility in DE, offers an experienced and tenured senior management team, and serves New Castle County and northern Maryland.

In this rewarding role, you will provide direct patient care to geriatrics, adults, adolescents and children with psychiatric disorders to ensure the safety and well-being of our patients.
Key Responsibilities include:
  • Assess patients through a systematic process of interacting with patient, patient families, and the appropriate health care providers during the admission of a patient
  • Collect and analyze data regarding the patient's physical, psychosocial, environmental, self-care, educational, and anticipated discharge planning needs
  • Reviews labs for patients' alcohol or drug toxicity, identify signs of withdrawal, and assess other medical complications that will require additional care at Rockford Center
  • Develop a comprehensive outline of care and specific goals and interventions to be included in treatment planning
  • Communicate changes in patient's condition
  • Participate in Hospital emergency codes necessary to the Unit's and the Hospital's safe operation
  • Comply with safety and security policies and procedures, including those regarding incidents and injuries
  • Complete Nurse-to-Nurse reports for incoming patients and review labs for Admissions when available
  • Complete the Nursing Admission when to include: Database, Treatment Planning, Flow Sheet, Admit Note, Medication reconciliation and admission orders

Qualifications
Job Requirements:
  • Education: Associates or Bachelor's Degree with an emphasis in Nursing. Strong communication skills, excellent customer service and organization skills, highly effective listening skills, and strong clinical skills.
  • Experience: Two (2) years of related experience, with knowledge of psychiatric patient care techniques with understanding of mental illness, preferred.
