Trinity Hospice is currently searching for a compassionate and experienced full time Chaplain with hospice experience for the South Austin service area, including communities like Buda, Kyle, and other southern towns. This is a newly-created role due to growth and to increase our team member's capacity. This person will primarily work evenings and weekends and will be paid per visit. 

Job responsibilities can include but are not limited to:

• Asses patient/family to identify spiritual needs and develops a spiritual plan of care.
• Collaborates with local clergy as needed.
• Conducts services of prayer, worship and ritual for patient/families, as appropriate.
• Provides individual patient/family counseling, crisis intervention, information and assistance with preparation of advanced directives, funeral planning issues and transfer of responsibilities relative to legal and fiscal issues.
• Work with IDT, participates in the ongoing POC by developing, documenting and evaluating responses to interventions in order to meet spiritual needs of the patient and family.
• Provides consultation, education, and support to the interdisciplinary team on spiritual care and bereavement counseling.
• Makes visits to patient/family prior to death and will be available to family during the bereavement period as needed.
• Comply with applicable legal requirements, company standards, policies and procedures

Qualifications:

• Qualified candidates will have at least one year of experience as a Chaplain in a hospice setting
• Masters of Divinity preferred
• Must have a current Texas Driver's license and dependable transportation
