Primary responsibilities will be:

• Providing exceptional care in coordination with the assigned nursing care team
• Maintaining patient records that include any changes in doctor’s orders, diet or personal living activities
• Help with patient meals and feeding, as well as basic activities of daily living
• Deliver weekly reports to the case manager and review any important changes in the patient’s condition
• Educating patients and family members
• Preparing timely, complete and accurate clinical notes
• Communicating timely and professionally with healthcare providers and other care team members

Must-have’s to be considered for this position:

• CNA license in the State of Texas
• 1 years Home Health experience
• Ability to lift up to 50 pounds
• Ability to work occasional weekends, holidays, evenings
• Exceptional customer service skills
• (Preferred but not required) Experience with Home Care Home Base EMR
