Job Description:

We are seeking a fun and creative part-time/PRN Occupational Therapist (OT) in  the Toledo (Sylvania), Ohio area providing services within an Assisted Living environment.

The Occupational Therapist (OT) would be responsible for performing starts of care, initial evaluations, performing subsequent follow-up visits, and supervising COTA's.  Must be a licensed Occupational Therapist (OT) in the State of Ohio . We utilize a universal electronic documentation system. Prior home healthcare experience is preferred, but not required.

Compensation : competitive/negotiable/experience-based. This position is per-visit rate with each visit type allotted its own rate.
