Seeking LPN for Outstanding montessori based memory care
The duties and responsibilities of the LPN Charge Nurse include providing direct nursing care to residents in an ALF unit under the direction of the Administrator and supervise the day-to-day nursing activities performed by nursing assistants, med techs, RAs and third party providers and others, as directed. Also to observe and report symptoms and conditions of residents/patients, assist in administering treatments, as prescribed by a physician and maintain records reflecting resident condition and treatment. Identify nursing service problems and assist in their solution and cooperate with other members of the nursing team and personnel of other departments. Assign duties to non-professional nursing personnel, supervise and evaluate work performance, and assist in maintaining a physical, social and psychological environment that is conducive to the interest and welfare of residents. Such care, supervision, etc., will be in accordance with the established policies and procedures at The Oaks, to maximize fulfilling the care-giving needs of residents. The person(s) holding this position will be delegated the responsibility for carrying out the assigned duties and responsibilities.
ESSENTIAL FUNCTIONS OF THE JOB
The following job functions have been determined to be essential to the position of LPN Charge Nurse. Because of fluctuations in workload demands, as well as tasks often overlapping, frequency ranges and percentages vary by assignment, day of week, day, evening or night shift,
etc. However, low frequency and percentage or duration of time spent on a particular task may not indicate lack of importance. The Oaks reserves the right to modify this list of essential and other functions, as deemed necessary.
Job Title: LPN Charge Nurse (continued)
Give and receive condition report from the on-coming and off-going shift.
Give direct nursing care to residents.
Direct the day-to-day functions of AL staff and other staff being supervised.
Give treatments to residents per house policy and physicians orders. Visit all assigned residents at regular intervals during tour of duty.
Notify physician when residents condition warrants.
Perform administrative duties such as the documenting of Nurses notes Doctors orders, discharge plans, reports.
Resident Assessment and Comprehensive Care Plans
Billing and other charge slips
Miscellaneous forms, charts, etc.
Chart pertinent facts and information in a professional, clear and comprehensive manner.
Admit, transfer and discharge residents, according to policy.
Recognize and respond to changes in residents conditions and document observations, interventions and outcomes.
Monitor and/or record follow through for signatures of telephone orders from physicians.
Conduct and/or monitor prescribed treatments.
Perform emergency procedures such as Cardiopulmonary Resuscitation and, in the event of death, notify family and funeral home.
Recognize, intervene and report accidents and incidents, when they occur.
Monitor and update care plans, each shift, as appropriate.
Supervise and participate in fire safety and other disaster drills, and be prepared to carryout duties, in the event of an actual fire or disaster. Recognize, intervene and/or report complaints and grievances from residents, families, visitors and employees.
Monitor the unit for possible health and safety hazards.
Maintain privacy and confidentiality of records, conditions and other information relating to residents, employees and facility.
Follow the established universal precautions and isolation procedures. Notify supervisor of pending absence or tardiness within the time frame described in the personnel handbook.
Encourage an atmosphere of optimism, warmth and interest in residents personal and healthcare needs.
Follow established smoking regulations and report violations. Perform other related duties, as assigned by supervisor.
OTHER
In addition to the essential job functions described above, the following job functions are important to the proper fulfillment of the duties of LPN Charge Nurse.
Answer telephone as appropriate.
Update daily census reports and submit as required.
Attend and participate in orientation programs, on-going training and educational classes.
Inventory and request equipment, supplies, etc.

Order from pharmacy, prepare and administer medications as ordered by physician and assigned.
Assist in updates of written policies and procedures of routine nursing care.
Participate in surveys made by authorized government agencies.
Complete remainder of medical forms, reports, evaluations, studies, charting, etc., not considered as essential functions.
Admit, transfer, discharge residents.
Monitor and participate in daily clean-up of the nurses office.
Perform other related duties as assigned by supervisor.
EDUCATION REQUIRED
The LPN Charge Nurse will have received a nursing degree from an accredited nursing program, college or university.
LICENSURE/CERTIFICATION
The LPN Charge Nurse will hold a current, unencumbered license to practice as a nurse in the State of Florida.
EXPERIENCE
The LPN Charge Nurse will have at least six (6) months experience working in a hospital, long-term care facility or other healthcare institution with training or experience implementing total patient care and care planning and in making sound nursing judgments. In addition, the LPN Nurse will be familiar with the policies, regulations and procedures of assisted nursing facilities. It is preferred that the LPN Charge Nurse will have taken courses in nursing management or supervision and in geriatric rehabilitative and restorative nursing.
