Job Description:

Be a part of a world-class academic healthcare system at UChicago Medicine as an Adult Inpatient Case Manager (RN). In this role you will partner with the rest of the clinical team to provide patients the highest quality of care. Our Care Coordination team delivers the highest quality of care while ensuring continuation of care for our patients is a seamless process.

The Case Manager (CM), a registered nurse with clinical expertise in a given area, plans and monitors consistent patient care delivery. Utilizing a clinical pathway or plan of care, the CM collaborates with the patient, family, the care team, and the multidisciplinary team members to assure safe, cost effective, timely and efficient care. The CM has the responsibility to evaluate the appropriateness of care and facilitate mid-course care changes, in conjunction with the patient care team and multi-disciplinary team members. Additionally the CM will be able to support, teach and act as a resource for all members of the health care team.

The Case Manager is a member of the leadership team charged with developing and maintaining patient care standards. They will coordinate and oversee the patient's care primarily during the inpatient phase of treatment. The Case Manager will collaborate with other members of the care team and multidisciplinary team as dictated by patient need and appropriate utilization of resources.

Essential Job Functions:

  • Clinical
    • Plans, monitors, and coordinates patient care using the plan of care or clinical pathway.
    • Identifies patient and family needs for consultation with other multidisciplinary specialists and assures needs are met.
    • Plans and conducts or participates in patient care conferences.
    • Assesses patient discharge needs. Assures completion of all discharge related functions on a timely basis.
    • Provides consultation to staff nurses in evaluating current patient status and plan for care.
    • Assures that comprehensive information necessary for the documentation of the patient's hospital course is available to all care team members. Documents in the patient record as appropriate.
  • Leadership
    • Evaluates the appropriateness of care in collaboration with the care team and the multi-disciplinary team. Provides direction to others in achieving any necessary changes.
    • Collaborates with all hospital departments and other disciplines to streamline services and effectively monitor utilization of resources. Prepares written reports and oral presentations of aggregated variances and cost effectiveness as requested.
    • Complies with standards and regulations of licensure, regulatory and accrediting agencies.
  • Education/Research/CQI
    • Participates in all Continuous Quality Improvement processes: assists in determining issues to be examined, participates in data collection and analysis.
    • Assists in initiating and implementing necessary changes in patient centered care delivery through the sharing of research, current trends, and authority as an expert practitioner.
    • Promotes utilization of research findings by making appropriate recommendations for change in policies, procedures, and practice.
  • Professional Development
    • Assumes responsibility for maintaining own competency and continuing education in area of specialization and in professional practice.
    • Participates in educational offerings.
    • Disseminates professional contributions in lectures, seminars and publications.
  • Performs other duties as assigned.

Required Qualifications:

  • A Bachelors degree in Nursing from an NLN accredited school of nursing
  • A current Illinois Registered Nurse license
  • 3 or more years of Nursing experience

Preferred Qualifications:

  • A Masters degree in Nursing from an NLN accredited school of nursing
  • 1 or more years of leadership experience
  • Prior experience in Case Management
