Job Description:

Be a part of a world-class academic healthcare system at UChicago Medicine as a Program Coordinator at our main medical campus. In this role you will partner with the rest of the clinical team to provide patients the highest quality of care. Our Care Coordination team delivers the highest quality of care while ensuring continuation of care for our patients is a seamless process.

The Program Coordinator performs advanced/specialized physical or occupational therapy or exercise physiology services/procedures and consultations, primarily in area of advanced clinical competency and/or specialty certification. This position develops clinical programs within area of expertise and provides training to other staff and students. The Program Coordinator is responsible for oversight and development of the assigned program. The compliance of the program with all regulatory requirements, the marketing of the program, and the initiation of program protocols and procedures will be the responsibility of this position. The direct management of employees assigned to the program and other resource allocations will

be done by the Assistant Director of the area, with input from the program coordinator. The Program Coordinator will be responsible for all post graduate trainees/students assigned to the Program, the mentoring of staff with compliance of performance and competency validation, maintaining program certifications and the approved research/performance improvement projects within the program.

Essential Job Functions:

  • Evaluates patients utilizing the advanced/specialized test and measurements; to determine the problem/level of function and the expected discharge environment.
  • Promotes and has knowledge of and utilizes appropriate patient care protocol relating to the physical,psychosocial and developmental needs of neonatal, pediatric, adult, and geriatric patients.
  • Establishes a treatment plan based upon the evaluation findings; and the patient/family, the referring practitioner and clinician's goals.
  • Instructs patient/families in home programs.
  • Performs advanced/specialized physical or occupational therapy or exercise physiology techniques and procedures in the area of expertise. Treats/consults on patients with complex problems i.e. co-morbidities and/or poor previous outcomes. Documents findings; patient progress; and procedures performed, and time required.
  • Demonstrates effective time management; achieves designated productivity standard. Attends rounds,conferences and clinics as assigned; reports on patient status and recommends patient discharge environment.

Qualifications:

  • Graduation from an accredited physical, occupational therapy or exercise physiology program
  • Bachelors or Master's or DPT degree or verification by the FCCPT /AOTA of equivalent training in another country or Exercise Science program.
  • Completion of an advanced degree in the field or specialty professional certification required as well.
  • Meets all the requirements of a level 3 for PT/OT (for PT/OT specific programs) Experience or training equivalent to that specified for an Exercise Specialist by the American College of Sports Medicine for EP (for Cardiopulmonary Rehabilitation program)
  • Specialty board certification and/or advanced clinical competency certification or advanced degree beyond entry level in field of Physical or Occupational Therapy or clinically related area, or completion of residency program in area of specialty. Requires at least 5 yr. clinical experience, of which a minimum of 2 years is in the related area of program expertise
  • Knowledge of Regulatory requirements for patient care, research agendas, and professional standards of care including supervision of staff).
  • Documented completion of research/ publication/ and/or national/state presentations Active participant in Professional Association, local, state or national. Is a member of APTA, AOTA or AACVPR

Licensure:

  • Exercise Specialist certification by the American College of Sports Medicine. (EP only)
  • Current physical or occupational therapy license in the State of Illinois for PT/OT roles
  • Advanced clinical Specialty certification within program area BLS certification required.
  • ACLS required for PC for Cardiac and Pulmonary Rehabilitation program.
