Job Description
The Salvation Army Mission Statement:

Job Objective:
The Supportive Housing Case Manager provides strengths-based case management utilizing the Pathway of Hope Case Management Model to maintain housing, provide resources, and preventing a return to homelessness.
What You Will Do:
  • Provide strengths-based case management to individuals and families residing in a residential program.
  • Provide Pathway of Hope case management.
  • Provide short-term and crisis intervention.
  • Advocate for residents regarding landlord/tenant guidelines, property management, and collaborating agencies.
  • Complete statistical and data reporting as required by The Salvation Army and funding sources.
  • Coordinate and/or facilitate life skill courses for fiscal management, parenting skills, and offer focused group Bible Studies, and other topics.
  • Develop individualized educational, health, nutrition, and family support plans for all participants for the purpose of furthering family goals, plans, and success strategies; provide parenting classes/workshops and develop action plans to improve healthy living and family dynamics.
  • Oversee and facilitate in partnership with the Family Ministries Coordinator all spring/summer/fall/holidays' children activities/camp including residents at the WCC, Barton Apartments, and Carpenter Center; may provide transportation for parents and children.
  • Participate in community networking committees supporting housing and program development.
  • Provide summaries for the Advisory Council with The Salvation Army Women and Children's Center.
  • Collaborate with The Salvation Army Women and Children's Center's on-going activities and be part of the On-Call call staff as needed
  • Participate in regular supervision.
  • Participate in compliance and territorial reviews.
  • Maintain clear communication with the property management company.
  • Support residents in ministry and development of pastoral care.
  • Other tasks as assigned by leadership.
Minimum Qualifications: The requirements listed below are representative of the minimal education, experience, skills, and/or abilities required for this position.
Education: Bachelor's degree in social work, psychology, sociology, or related field.
Background Checks: The position requires a background check to be completed. Findings may disqualify an individual for this position
Experience: Two years providing case management and support services in a social service setting. Assisting vulnerable populations is preferred. Knowledge of Housing and Urban Development standards preferred.
Certifications: Must have a valid driver's license and maintain The Salvation Army Driver's qualification standard; must complete Safe from Harm training within the first 90 days of employment; completion of Case Management Certification Program within 120 days of employment; completion of SIMS data training within 60 days of employment.
Skills/Abilities:
  • Able to speak, write and understand English in a manner sufficient for effective communication with leadership, field personnel, and clientele.
  • Computer proficiency with Microsoft products and ability to learn electronic reporting systems.
Supervisory Responsibility: No supervisory responsibilities

Physical Requirements Include:
  • Good speaking, hearing and vision ability, and excellent manual dexterity.
  • Lifting, pulling and pushing of materials up to 25 pounds.
  • May require bending, squatting, walking.
  • May require standing for extended periods.
Travel: May require travel for training and local travel for meetings.
Working Conditions: Work is performed in an office environment located in a residential building; may require some weekend and evening work.
