Position OverviewThe Medicare Annual Wellness Visit Nurse is responsible for implementing Annual Wellness Visits and Chronic Care Management services at their assigned PHN sites. Annual Wellness Visits for Medicare patients consist of Initial Preventive Physical Exam visits (IPPE or “Welcome to Medicare”) and Annual Wellness Visits (AWV) both initial and subsequent. The Primary Health Network began as one small community health center site in the Shenango Valley in 1984, with a central focus on providing the highest quality medical care to the people we serve.
