JOB SUMMARY
To supervise and provide quality nursing care to meet resident’s needs, coordinate, and conduct activities of daily living, to address family/social concerns, document and contribute to health status assessments and care plans, administer medications, and performs skilled treatments and procedures.

ESSENTIAL JOB DUTIES AND RESPONSIBLITIES

  • Provide daily supervision to assigned staff and directly provides nursing care to residents in Skilled Nursing Unit as assigned.
  • Completes the admission/transfer/discharge process for residents in its entirety as per facility guidelines.
  • Assists residents with Activities of Daily Living (ADL), family/social activities and actively promotes independent lifestyle of residents.
  • Responds to residents call system promptly and courteously.
  • Responsible for identification of resident medical and behavioral changes and the development and monitoring of care plans and reassessment of each resident.
  • Provides daily report to the Director of Nursing concerning facility conditions, personnel performance, resident conditions, and needs for nursing supplies.
  • Consistently demonstrates hospitality behavior and the core values of quality, respect, service and integrity.
  • Provides nursing care according to physician’s orders and in conformance with recognized nursing practice, established standards, and administrative policies.
  • Ensures preventative, therapeutic, and rehabilitative services to residents and assists in the coordination of nursing care with care rendered by other disciplines.
  • Makes rounds with physicians, assists in examinations and/or treatment of residents and carries out orders.
  • Has the skill to assess residents and notify attending physician and responsible party in the event of a change in the resident’s condition.
  • Accurately administers prescribed medications, supervises the continuity of nursing care, responds to changes in medical condition, medications or other circumstances requiring special care needs and performs first aid as required.
  • Provides daily supervision and leadership to assigned staff and other designated personnel in meeting the resident’s needs. Demonstrates good decision-making skills.
  • Assesses and monitors complex resident health status and determines the appropriate nursing intervention. Develops and monitors the ongoing plan of care with residents and initiates corrective action as necessary.
  • Orients residents and staff to emergency procedures and responds to all emergency situations.
  • Completes and signs incident reports and provides follow-up as required.
  • Assures that all resident records are accurate, complete and all mandatory forms are on file. Through familiarity and application of facility policies and procedures, assures that nursing care is practiced in full accordance with regulatory requirements, facility quality standards, and resident rights.
  • Monitors resident environment to ensure all safety and sanitary regulations and quality standards are met or exceeded. May assist with development and presentation of in-service programs.
  • May serve on committees, as requested.
  • Works on the assigned unit as per scheduling needs.
  • Document patient interaction and care appropriately in Electronic Medical Record as per facility policy and state guidelines
  • Follow directives from the immediate supervisor and communicate necessary information to the Director of Nursing
  • Responds to patient/family calls in a timely manner and escalates to Administrator or Director of Nursing if deemed appropriate.
  • Ensure resident rights and dignity are safeguarded in all aspects of resident care.
  • Attend mandatory nursing department meetings and training as schedule.
  • Works alongside with Certified Nurse aides to promote effective resident care.
  • Perform other duties as assigned.

QUALIFICATIONS AND EXPERIENCE
  • Licensed Practical Nurse with a current State of Illinois license.
  • Must be able to read regulations and write clearly.
  • Must be able to communicate in courteous, friendly manner.
  • Ability to effectively manage and direct C.N.A. working alongside for the day
  • CPR Card preferred
  • Must be able to lift up to 50 lbs
