Veteran Directed Care (VDC) Coordinator Job Summary

Who are we? NILP is a rapidly growing human service agency who assists persons with all types of disabilities who wish to live independently in their community throughout Northeast Massachusetts. NILP provides five core services of Advocacy, Peer Counseling, Information and Referral, Independent Living Skills Training, and Transition. NILP believes in consumer control and self-direction and provides these services with the knowledge that persons with disabilities are the best determinants of their needs and goals.

Position Overview : This position is responsible for coordinating services for consumers enrolled in the Veteran-Directed Care (VDC) Program. The VDC program provides veterans with opportunities to self-direct their long-term services and supports to continue living independently at home. Eligible veterans manage their own flexible budgets, decide what mix of goods and services best meet their needs, and hire and supervise their own workers.

What you will be doing:

• Provide timely and accurate assistance with referrals to support the consumer in accessing the agreed upon long-term supports.
• Develop and manage a tracking system for new program referrals.
• Process and outreach to new program referrals in a timely manner.
• Provide guidance and training on rules and regulations regarding the VDC program to eligible veterans.
• Assist the veteran in developing a plan that is self-directed and includes goods, services, financial resources, and other supports agreed upon by the veteran.
• Travel out into the community to visit with veterans in their homes on a quarterly basis to monitor program progress and answer questions.
• contacts to facilitate the veteran securing needed services and supports.
• Other duties and special projects as assigned.

Who we are looking for?

• Bachelor's Degree or equivalent in Human Services or a related field. May be substituted with equivalent experience as a person living with a disability.
• Veterans are strongly encouraged to apply.
• At least 3 years' experience in human services, disability related services, or care coordination. Can be substituted with equivalent lived experience as a person with a disability or as a veteran.
• Knowledge of veteran related resources and services strongly preferred.
• Proficient skills with technology including Microsoft 365, Teams, mobile devices, and applications.
• Excellent interpersonal, verbal, and written communication skills
• Ability to travel independently & reliable transportation required
