General Description:
Private Duty Cases all on one Campus. Performs basic care procedures. Recognize the elderly we serve as unique persons, requiring individual alternatives to maintain full lives in their later years. Provide medical, spiritual and emotional care; encompassing quality, dignity, compassion and confidentiality for each individual.
Essential Job Functions:
1.Communicates with resident, families and staff in a sensitive, professional and therapeutic manner.
2.Observes and communicates changes in the resident condition to the RN responsible for the care of the client.
3.Assists in the residents activities of daily living which includes assisting with meals and nourishment, transferring using proper body mechanics and appropriate assistive devices, ambulation, bathing and dressing.
4.Documents care and observation on appropriate forms.
5.Provide for the safety and security needs of residents.
6.Maintains established department policies, procedures and objectives, quality assessment and improvement programs, and safety, environmental and infection control standards.
7.Attends meetings as required.
8.Participates in education programs and inservice meetings.
9.Understands and follows “Universal Precautions” as it pertains to potential exposure to all body fluids. Utilizes infection control techniques.
10.Reports all unsafe conditions to appropriate supervision, including change in resident status, broken or malfunctioning equipment or anything that could be a potential threat or danger to a resident. Practices good safety awareness.
11.Knowledgeable of Residents Rights and facility Abuse Policies and Procedures.
12.Performs any other duties and responsibilities that may become necessary or as directed by Supervisor.

Work Area and Working Conditions:
Entry Level Qualifications:
1.Post High School Vocational/Specialized training.
2.Current State of Connecticut CNA certificate/or HHA designation; CPR certified preferred.

Key Accountabilities (with or without reasonable accommodation):
1.Demonstrates the ability to provide personal hygiene for residents by bathing, oral hygiene, denture care, hair and nail care, skin care, incontinent care, dressing and undressing.
2.Demonstrate ability to provide nourishment by preparing meals, providing assistance in the resident dining room and serving nourishment.
3.Demonstrates ability to provide incontinent care to residents by application and disposal of diapers, use of bed pans and urinals, toileting resident with or without assistance and perri care.
4.Demonstrates safe transfer with wheelchair, walkers and other ambulation devices.
5.Demonstrates ability to recognize change in residents condition and notify the nurse.
