Job Description
POSITION SUMMARY
The clinical pharmacist will be a unit-based pharmacist that will verify the orders and provide clinical services for the area(s) for which they are assigned. They will be a drug information resource for the nurses, physicians, patients, and families. The clinical pharmacist will be expected to fulfill staff pharmacist responsibilities as needed.
QUALIFICATIONS
MINIMUM REQUIREMENTS
Education: Bachelor of Science Degree in Pharmacy required, Doctorate of Pharmacy Degree preferred.
Experience: Pharmacy practice residency or equivalent hospital experience (3 postgraduate years)
License: Current registered pharmacist license in the State of Florida
Certification: Pharmacy license; BLS within 30 days of hire (ACLS and/or PALS if code attendance required); Board Certification in Pharmacotherapy (BCPS) or other specialty corresponding with practice area.

Responsibilities
This individual's responsibilities include (but are not limited to): All general responsibilities of a Staff Pharmacist. Provide direct patient-centered care and integrated pharmacy services in collaboration with physicians, nurses, and other healthcare professionals. Routine attendance on medical rounds (for decentralized shifts) with consistent documentation of pharmacist interventions. Deliver medication therapy management and ensure pharmaceutical care programs are appropriately integrated throughout the institution. Assist in development and implementation of facility-specific protocols, guidelines, and reference documents related to medication therapy management. Conduct Medication Use Evaluations (MUEs) and assist in making necessary changes to pharmacotherapy practice as needed based on MUE findings.

Physical Demands:
Lift/position up to 50 lbs. Push/pull up to 50 lbs. of force. Frequent standing and walking. Frequent reaching, stooping, and bending. Manual dexterity, mobility, touch, auditory to perform all the related duties of the position.
