Who were looking for:

This individuals responsibility will be to provide case management to clients in one or more program(s). These services may include, but are not limited to:

  • Education
  • Medical Care
  • Economic
  • Vocation
  • Prevention
  • Housing
  • Aftercare
  • Intake/Assessment

In addition, this person may handle an independent caseload and assist other personnel by ensuring proper referral of clients to all appropriate services available and necessary to them.

Work is performed in accordance with established regulations, policies and procedures, but the person is expected to exercise considerable initiative and independent judgement in achieving goals.

Responsibilities:

Your responsibilities in this role may include:

  1. Establish and maintain referral relationships with service providers
  2. Arrange medical referrals for clients when applicable.
  3. Arrange DPA, social security, food stamps and medical assistance appointments to ensure timely and continued receipt of benefits.
  4. Arrange psychiatric evaluations and medication when appropriate.
  5. Identify appropriate social service resources for clients and refer clients as appropriate.
  6. Refer clients to appropriate vocational and educational programming.
  7. Conduct outreach efforts to maintain full caseload and facilitate referrals to residential program
  8. Maintain contact with discharged clients until they have been established with other case management/social services.
  9. Maintain valid drivers license.
  10. Supervise and train case managers.
  11. Oversee case management for assigned unit.
  12. Other related duties as assigned by Supervisor, and as described in the Unit Unique Job Description

Qualifications:

Minimum Requirements

Education/Training: Bachelors degree in a generally recognized clinical discipline, plus specialized training for specialized population; and/or full certification as an Addictions Counselor; or Masters degree in related specialty area

Experience: Four (4) years of D&A counseling experience or case management.

Required Knowledge Skills, and Abilities

  • Specialized training experience in specific population, i.e. homeless, HIV positive etc.
  • Knowledge of the basic principles, practices, and procedures involved in an alcohol and drug program.
  • Knowledge of the drug and alcohol abusing subculture.
  • Knowledge of the physical and emotional characteristics and manifestations of alcohol and drug
  • Knowledge of individual and group behavior and the effective ways of working with them.
  • Ability to work effectively with people to aid them in utilizing available resources.
  • Ability to establish and maintain effective working relationship with clients, and other members of
    staff, outside agencies and institutions, and the general public.
  • Ability to communicate effectively, both verbally and in writing.
  • Ability to initiate and maintain contacts with social service agencies, vocational programs, educational
    programs etc.
  • Ability to organize a large caseload and an extensive informational system.
  • Ability to train and supervise employees.
