Our mission is to improve the livesof those we serve, and we are seeking a Part-Time Field Nurse Supervisor(RN) to join our team.As a Field Nurse Supervisor for our Licensed Home CareService agency, you will play a crucial role in ensuring the delivery ofhigh-quality home care services in compliance with the regulations set by theNew York State Department of Health. This per diem position combines clinicalexpertise with managerial responsibilities, overseeing various aspects ofpatient care, caregiver competency, and documentation. This role is vital tothe continued growth of Synergy HomeCare and will help shape how weinterrelate with our Caregivers and other senior-focused agencies.
Key Responsibilities: 
  • Ensures the qualityof patient care and service by using specialized knowledge and judgment to ensurethat professional standards are in compliance with applicable State PracticeActs and Agency policies.
  • Coordinates withhome office staff to ensure the appropriate assignment of personnel to meet thespecific needs of an individual client.
  • Builds and maintainsrelationships with clients, community agencies, and professional organizations.
  • Maintains compliancewith all agency clinical and administrative policies, procedures, andregulatory requirements.
  • Knows and interpretscommunity resources available for continuity of patient care.
  • Investigatesproblems, irregularities, policy violations, and other reportable eventsimplementing the appropriate corrective action and follow-up.
  • Schedules andconducts initial nursing assessments for new patients.
  • Performsreassessments as required to determine the ongoing needs of patients.
  • Conducts Home HealthAide (HHA) supervisory visits in accordance with agency policies.
  • Ensures caregiversare providing care in accordance with patient’s care plan.
  • Investigates andfollows up on incidents or clinical issues.
  • Collaborates withthe office team to address and resolve concerns.
  • Completes allrequired nursing documentation associated with patient visits.
  • Submits medicalorders, interim orders, incidents, accidents, and orientations, as necessary.
  • Develops, updates,and maintains comprehensive patient care plans.
  • Ensures care plansaccurately reflect the patient’s ongoing needs.
  • Coordinates with theoffice team to ensure seamless communication and implementation of patient careplans.
  • Provides necessaryinformation for scheduling and coordination of services.
  • Spends a significantamount of time visiting patient’s homes and medical centers.
  • Monitors thedelivery of care and addresses concerns in real-time.
  • Promotes Agency tothe community, participating in community programs with other professionalswhen necessary.
  • Providesorientation, in-service education, and additional training for Home HealthAides, Registered Nurses, and Professional Staff Personnel.
  • Ability to work on apart-time/per diem basis.
  • Other duties asassigned by supervisor.
Qualifications:
  • Current RN licensein good standing.
  • BSN/MSN.
  • Minimum (2) twoyears of recent and verifiable clinical and managerial experience within a HomeCare setting.
  • Cheerful personalityand excellent communication skills.
  • Willing to meet andspeak with patients, their families, and their medical professionals todevelop, update, and maintain proper care plan.
  • Strong assessmentand organizational skills.
  • Prior supervisoryexperience with field staff, and experience with clinical oversight, monitoringquality assurance, and overall case management.
  • Demonstratedsuccessful track record of working independently.
  • Ambition to grow andadvance beyond current position.
  • Solid communicationand PC skills.
